Predicting unplanned hospital readmissions
نویسنده
چکیده
The number of unplanned readmissions is an important quality improvement indicator for Australian hospitals, as it has a direct impact on costs and patient outcomes. It is a useful indicator for patient multi-morbidity to identify patients that should potentially have different treatment or discharge plans. The objective of the Health Insights Challenge project was to develop a predictive model to determine the likelihood of an unplanned hospital readmission. The outcome was a validated analytical model that predicts the likelihood of an unplanned hospital readmission occurring within 30 days after a given in-patient episode. An ‘all-cause’ model does not pay attention to any specific disease or diagnosis groups. The predicted likelihood of an unplanned readmission occurring can assist care workers and clinicians in planning treatment and discharge processes.

Assessing the importance of predictors in unplanned hospital readmissions for chronic obstructive pulmonary disease
PURPOSE The all-cause readmission rate within 30 days of index admissions for chronic obstructive pulmonary disease (COPD) was approximately 21% in the United States in 2008. This study aimed to examine patient and clinical characteristics predicting 30-day unplanned readmissions for an initial COPD hospitalization and to determine those predictors' importance. PATIENTS AND METHODS A retrospe...
متن کاملUnplanned readmissions to acute care from a pediatric postacute care hospital: incidence, clinical reasons, and predictive factors.
OBJECTIVE To identify the incidence, clinical reasons, and predictive factors for unplanned readmissions to acute care from a pediatric postacute care hospital. METHODS A retrospective cohort analysis of all discharges between October 1, 2011, and September 30, 2013 (n=298), in 1 pediatric postacute care hospital was conducted. Descriptive statistics were used to summarize the incidence and a...
متن کاملAcute kidney injury as an independent risk factor for unplanned 90-day hospital readmissions
BACKGROUND Reducing readmissions is an international priority in healthcare. Acute kidney injury (AKI) is common, serious and also a global concern. This analysis evaluates AKI as a candidate risk factor for unplanned readmissions and determines the reasons for readmissions. METHODS GLOMMS-II is a large population cohort from one health authority in Scotland, combining hospital episode data a...
متن کاملProlonged beneficial effects of a home-based intervention on unplanned readmissions and mortality among patients with congestive heart failure.
BACKGROUND A single home-based intervention (HBI) applied immediately after hospital discharge in a cohort of "high-risk" patients with congestive heart failure has been shown to decrease numbers of unplanned readmissions plus out-of-hospital deaths during a period of 6 months. The duration of this beneficial effect remains uncertain. METHODS Hospitalized patients with congestive heart failur...
